Re: Health Records to be Destroyed- June 2024

Per Connecticut State Law (34 CFR §300.573) School officials at the Litchfield Public Schools are planning to destroy the Health Records of students belonging to the graduating classes 2015 - 2017 in June of 2024. Additionally, the health records of those students who attended Litchfield schools with those classes, but did not graduate, will also be destroyed. Confidential records are destroyed six years or after they become irrelevant to the provision of educational services or after graduation. This does not include immunization records.
Anyone interested in receiving their records should make the request by emailing the school nurse, Theresa Simaitis, at simaitist@lpsct.org or by mail at 14 Plumb Hill Road Litchfield, CT 06759.
It is preferred that you pick up your records in person; however, please note that if you are unable to appear in person, a signed notarized letter indicating who will be picking up your records must be submitted.
