Hello Parents/Guardians Grade 6 Students,
A friendly reminder: A complete physical exam with updated immunizations is required for all 6th-grade students. I must have the completed "blue" physical form on file prior to your child's last day of 6th grade. The blue State of Connecticut Health Assessment Form must be fully completed by your child's healthcare provider with all required 6th-grade immunization dates noted and attached. If your child has immunization exemption(s), I must have an updated form from your child’s healthcare provider. The front of the blue form is to be completed and signed by a parent or legal guardian. Please return the completed blue form and immunization record to me as soon as possible and before the last day of this school year. Now is not too early to make an appointment with your child’s healthcare provider.
